How It Works

1. Call 1-800-800-7616 or find a list of participating providers in your area here. (Note: Be sure to have your Member ID and Group ID, from the front of your membership card, before calling.)
2. Present your membership card (with the Coast to Coast Vision logo) at participating locations to receive savings on your procedures and vision products.
3. The total discounted fee must be paid at the time service in order to receive a discount.
4. Access discounts on contact lenses through America’s Eyewear and eyeglasses and sunglasses through FramesDirect.com.

Coast to Coast Vision^TM is not available in Vermont. Coast to Coast Vision^TM providers may exclude the following from discounts:

• Exclusive frame lines like Cartier, which contractually prohibits a retailer from discounting their products
• Marketing Promotions/Sales
• Promotional Package Pricing
• Disposable Contacts (Cole Vision Retail Stores offer a 10% discount on disposable contact lenses. Members may also purchase disposable contacts through the mail order service.)

With a $15 visit fee and 24-7 access, save time and money by connecting with board-certified doctors by phone or video – anytime, anywhere. Doctors offer a diagnosis, treatment options, and even prescription when medically necessary.

• Get treatment for common medical issues such as colds, flu, poison ivy, respiratory infections, bronchitis, pink eye, sinus problems, allergies, urinary tract infections, and ear infections
• Average doctor response time is 10 minutes
• Visits for all ages—from children to seniors
• U.S. board-certified doctors with an average 20 years practice experience
• Upon member request, Teladoc can share visit information with your doctor

How It Works

1. Prior to using this service, register here or call 1-800-800-7616
2. Each member must provide Medical History online or by phone, similar to paperwork completed at the doctors’ office; it takes 10 minutes to complete. Note: be sure to have your member ID and Group ID, from the front of your membership card, before calling)
3. Once this is complete, you may go online or call to schedule a visit (a physician will call the member back within three hours, in most cases less than an hour)
4. Repeat steps for each participating family member.
