Event Liability – Request a Certificate of Insurance
When you need proof of insurance for your event, send an email request containing the information listed below to Luz Maysonet of AMBA to obtain a Certificate of Insurance.
- Submit a copy of the insurance request or contract from the Certificate Holder to AMBA.
- Event description, location, and dates.
- Name and address of the Certificate Holder.
- Does the Certificate Holder contractually require you to list them as an Additional Insured?
- If yes, provide the exact language required.
A Certificate of Insurance will be sent only to ASUC.
